Click for New Patient Forms Initial Appointment Prior to your first appointment, please download, complete and send Dr. Mote a copy of your new patient packet which includes details of your health history, pertinent family medical history, lifestyle factors, and a 7 day diet diary. The initial consultation will involve a thorough review and discussion of your personal health history. With this information, Dr. Mote will make recommendations for specific laboratory tests necessary to identify and address your specific health concerns. This appointment is typically 45-60 minutes in length. Test Kits Recommended test kits will be mailed to your home address. Once the test samples have been collected you will send them to the laboratory for processing. The laboratory will deliver your test results to Dr. Moteâs office (processing time varies from 2-4 weeks depending upon the type of test). At this time our office will call you to schedule a detailed report of findings appointment. Report of Findings Appointment and Follow-Up Appointments During the report of findings appointment, Dr. Mote will explain the meaning of your test results and recommend a treatment program typically consisting of diet, sleep or exercise changes. Additionally, nutritional supplements, medical treatments or further testing may be discussed. Subsequent visits are scheduled to monitor your progress and help you stay on track with your individualized health program, and may vary from 15-20 minutes. We encourage you to schedule appointments a week in advance. Insurance Cornerstone Health Services is not affiliated with any insurance plans. It is the patients responsibility to determine whether their insurance carrier provides coverage for consultations, tests, and supplements. At you request, we will provide a superbill to you that lists CPT and diagnosis codes that you may submit to your insurance provider. The superbill contains the codes for consultations and lab tests only. Most insurance companies will not reimburse for supplements. Medical Emergencies Dr. Mote is not a medical doctor; he does not service medical emergencies. If you have a medical emergency, you must contact your primary care physician or dial 911. You may also request the New Patient Forms be mailed to your address. - Dr. Mote works with patients within the continental US. If you live outside of the US, please contact us and Dr. Mote will direct you to a physician who can help you in a similar manner.
